Job Description

Position:  Mental Health Case Manager (Sylvan)                                       Position Category:  Professional

Report to:  Director of Permanent Housing or Supportive Services Manager

Responsibilities:

  • Conducts screenings and interviews of potential residents;

  • Serves as a liaison between participant and the mental health community (i.e. mental health providers, psychiatry, etc);

  • Provides general case management and referral services to all residents needing such assistance by establishing links with appropriate agencies and service providers in the community, to assure flexible and creative services for residents;

  • Assesses resident barriers that have kept them homeless and develop a service plan with housing goals and where applicable vocational goals and implement a plan of action;

  • Conducts needs assessment to address the emotional needs of the resident.  This would include counseling and referral to appropriate resources and services.  Develops case plan in coordination with community assessment services;

  • Monitors the ongoing provision of services from mental health community agencies and keeps track of progress of the individual, provides communication with all appropriate persons and agencies;

  • Keeps accurate records of visits (home and office) with residents; documents progress toward goals;

  • Attends supervision and participates in staff meeting, inter-agency or other coordinating meetings;

  • Attends community meetings and represents the Achor Center.

  • Prepares necessary correspondence, forms, documentation, reports, etc; files documents, reports and records as appropriate;

  • Enroll residents into the Homeless Management Information Systems (HMIS) in Pathways and maintain case management records in Pathways in a timely manner within 24 hours.

Qualifications:

  • Bachelor’s degree in social work or other human services field.  Prefer 3+ years related to experience.

  • Mental Health experience preferred.

  • Requires excellent oral and written communication skills and interpersonal skills.

  • Ability to multi-task and work independently.

  • Self-starter, highly motivated, creative thinker.

  • Accuracy and attention to detail

  • Must possess excellent computer skills (Microsoft Office, PowerPoint, Access, Excel, Work and Outlook
